A cover has to work at thumbnail on a phone, at full size on a shop table, and at 6mm spine width on a shelf. It also has to tell the right kind of reader this book is for them. We design for all of it.
We read the book (or the synopsis and three sample chapters) before drawing anything. Cover design that doesn't know what's inside is just packaging.
Most discovery happens at 200px wide. We design at that scale first, then scale up — not the other way round.
